Customers of Insight Communications and AT&T who lost telephone, cable-television or Internet service in last month's windstorm may qualify for a refund

Insight is accepting requests by mail, spokeswoman Reba Doutrick said. Submissions should be addressed to Insight, 4701 Commerce Crossing Drive, Louisville, KY 40229, with "Ike" on the left side of the envelope.

Applications should include the customer's name, address, phone number and account number, if available, and should specify the days without service, she said.

AT&T "will credit a customer for the days or time period" he or she lost phone service due to storm damage, spokeswoman Cathy Lewandowski said in an e-mail. Customers may call AT&T Kentucky at (888) 757-6500.
