So far, all that was needed to upgrade to Boost Plus was going to Walk-In CableVision store and obtain new Arris TM822G D3 modem, hook it up and provision it via web page. All 5 channels bonded.
However, since this is an "upgrade" don't expect blazing speeds for 24hrs as per verbal OOL tech support as it can take upwards of 24hrs to fully associate modem to account and enable the Boost Plus.
Finally got level of service, 50MB+ down/8 upload. Verified by connecting modem directly to CV drop and laptop to modem.
Problem was Boost Plus was not fully provisioned to both modem and account. Once done, with Arris TM822G now working fine.
