I've set up with a 12 month subscription for unlimited Skype calls within the US and Canada, along with a telephone number so that others can call me, for $60 a year. You can purchase a stand-alone phone and then use Skype as a home phone replacement. It has been great way to keep from eating up cell phone minutes when staying in touch with friends from college or during telephone meetings (I work from halfway across the country doing website administration and have lengthy phone meetings every few weeks).
Skype's call quality, while impacted by your internet connection quality and hardware, is generally very good, and is better than the voice quality over POTS. I purchased a stand-alone Skype phone, which looks exactly like your standard wireless handset, and have been very happy with the service.
Note, Skype does not provide 911 service, so if that is a critical feature for you, or you do not have a cell phone for emergency calls, this is not the right service for you.
