I am a Verizon FiOS customer, and have been for several years. I signed up for service before Verizon even laid fiber optic cables in the area. Less than two months after I signed up, they delivered my 5MB/s down by 2MB/s up. The installation was free, with the exception of the cabling from the ONT to my desk, which was fine since I ran my own Cat5e ahead of time. I originally contracted for 2 years at $ 25 per month, but have since been bumped up to 35 after taxes, still at 5x2. Verizon sales people have been pounding the area hard to get subscribers on the 3 pack ('Net, TV, Phone).
Others in the area have indicated the current prices for the same service is about $10 to 20 higher, or about 45 per month for 5x2. Also available in my area are faster packages, including a 30 Mbps/15 Mbps tier.
Because of a wind storm, apparently a tree branch hit and damaged the fiber optic line, causing my speeds to fluctuate and stop. It took two separate on site service attempts to isolate the problem, but Verizon ultimately figured out what happened and fixed it.
As a data only customer (I don't have phone service), I have found it crucial to have my data circuit ID number when speaking to anyone with Verizon over the phone.
