We have the Premium cable package, unlimited phone, and 50 mbps internet. We also have 1 HD DVR and 3 HD boxes. Update (01/02/2013): Signing a 2 year contract but only getting a 1 year loyalty discount is annoying to say the least. The good news is if you call them about it they will give you another 1 year loyalty discount when they first one runs out. It's just a hassle to call them for something they should do automatically. The channel changing bug where the box freezes for 5 to 30 seconds between channel changes is the only other issue we have right now. It's been there for like 6-8 months and if we're lucky they'll have it fixed sometime soon finally! Aside from the above two things everything else is pretty good.
