I totaly agree with most of your points. Pacbell DSL service is bouncing from "The best" to "the worst". Some of the reasons, I believe are that Pacbell techs are not as good as they think, technical support is not worth of criticism.
I have DSL 384/128 since January '99(1 static IP). I moved once since then, and here are some numbers.
I have been visited, in general, by 12 techs and have more then 10h phone conversations (to be read as "waiting on hold"), finally, after all, I have the service that I'm paying for.
I'm 10610 feet from the CO but my speed according to the "test tool" is about 530kbps down, and 114kbs up.
2 days ago I called Pacbell rep. and asked him to change my billing plan to 39.99 since I was paying 49.99. I found out that I can ask for refund for the 198$ installation and equipment fee that I payed in '99. I will post a message if I have any success with refund 
So, in summary, if you want to have DSL from Pacbell, consider that anything that you want to have you will have to fight for it.
If I have to rate Pacbell DSL in scale up to 5,I will give them 4, only because of the uninterupted DSL signal."
Added:
I have Alcatel 1000 ADSL modem, and I share the DSL line on 2 PCs and 1 Apple over a Intel hub. I'm using MS proxy 2.0 and is working without any problems.
