I work as a telecom system engineer, fancy talk for programming phone systems. Thusly, I have my own phone system in my home; Avaya IP Office. I didn't want to get traditional Analog trunks, and a PRI is a bit out of my price range, so I went with some fancy SIP Trunks.

A friend of mine had suggested Flowroute, and had already had a nice how-to written up on how to make it work with the system. Granted, it was a version or two behind, but with some mucking about, I made it work.

I put in my order for 1 DID, and within say 30 minutes, I had it all up and running. In a little over a years time, I've only ever 1 problem, and it was a major outage that affected some equipment on their side. They got it up and running, and zero problems since.
The billing is really easy. I just put in credit via amazon payments, and they withdraw it on per minute basis (unlimited inbound!), and charge me a few bucks per DID.

It's honestly been great. The quality is great, the service is great, the support is fantastic. I've deployed it to several customers and they seem to like it as well.

I've had to deal with *many* VOIP companies, touting great SIP. Flowroute is the only company that has delivered on it.
