I use exchangenet.net in corvallis oregon, and have been with them a little over two years. speed is roughly symetric with dowloads usually around 1 Mbps, which is pretty good actually considering how far I am from them. Cost is $50/month and there was no setup fee with a contract. I also have two static IPs. This is a very small business and they are willing to work with you to get what you need. If you need something unusual or just want a static IP this is where to go.
