Looks like it was a result of backdoor program (trojan), first getting in through your defenses, then getting gathered information out, again bypassing your defenses.
It could've been avoided by having anti-trojan software (or real time Anti-virus monitor with strong trojan detection) and firewall, capable of regulating outbound connections (WinXP firewall can't do this).
