I'm using a modified version from my web hosting provider. I believe the big concern is preventing people from modifying the recipient. I think the standard script has security features to restrict who the mail can be sent to.

I'm using the following fields:

•recipient•subject•required•env_report•print_blank_fields•return_link_url•return_link_title•bgcolor•text_color•link_color•vlink_color•alink_color•realname•email•comments

Let's start with a pointer to the page that is sending the mail. Maybe we can figure out what your markup and code is doing.
