looks like i am up. TB must have done something as I can no longer access the admin user with the slapshot pw
but i can make outgoing phone calls.

I moved to TB from SR last week. My void service was smooth last week even after transferring to TB. But once I reset my Gizmo(to turn off auto provisioning) I can no longer get my voip working. Run light is blinking red and void light is off. As I don't have passwd and auth info I couldn't follow all the steps in the first message. I tried telnet to Gizmo and start provisioning (Pr) but it's failing with the following message --

In case someone has the right provisioning and DMS proxy IP address for TB service please help me with those information. Calling TB is of no use as nobody picked it up even after waiting for 60 min.

I had the same problem as you, from following the TeleBlend FAQ which says to reset the gizmo to fix the flashing light condition -- bad idea! It didn't fix it and instead, service was down for almost 24 hours. I tried lots of things and finally gave up hope. I even initiated a port to another provider this afternoon.

However, it seems that TeleBlend service finally recovered on its own sometime tonight when I left it in the default 67.133.234.53:80 setting. Same with others I read about with the same problem. So hang in there; looks like it will recover itself with enough patience.

Funny thing with my "normal" state is that the voip light is on sometimes, off sometimes. When it's off, there's no dial tone, and yet it's intermittent enough that I can still dial out or receive calls -- then the light comes back on. Same behavior before and after that reset.

This is a hardware reset to factory default. This is from the INNOMedia Administrator Manual for the SIP MTA63228Re Manual version 4.2.16. Most routers have one.

In the back of the MTA press the "RSTR" button for 5 seconds. You need a paper clip to push the button. After 10 seconds the defaults will return to 192.168.99.1 IP Adress. The default "Admin" and "password".

Please remember - DO NOT STAY CONNECTED ON THE NET when you do this until you have turned off provisioning on your innomedia.

Sorry, I do not have matching information for the Linksys and SPA adapters... if someone does, please post it in this thread.

Here are some instructions for setting up an InnoMedia gizmo manually, from scratch, in this case using a server on an alternate port.

1. Unplug cable from gizmo WAN port (so you are disconnected from Internet).

2. With PC connected to gizmo LAN port, go to »192.168.251.1 , log in with user and welcome.

3. Open this URL: »192.168.251.1/restore2.ssi ; after about 15 seconds you should see a message about reset to defaults. Wait another 30 seconds.

4. Close browser window. At this point your gizmo should be in the same state as when SR first shipped it to you.

5. Open a new browser window, go to »192.168.251.1/Voice_adminPage.htm (do not open gizmo home page first).

6. When prompted for password, use user: admin and password: slapshot

7. Go to IP Network->Provisioning Setting. Uncheck Enable Provisioning. Click Save & Reboot. Click OK to warning.

8. When prompted, log in again with admin and slapshot . If you have trouble, close window, open another, go to »192.168.251.1/Voice_adminPage.htm

9. Gp to IP Network->Interface Setting. Re-enter your static IP or PPPoE information, if needed. Click Save & Reboot. Click ok to warning.

10. Reconnect gizmo WAN cable. WAN light should come on, and your Internet connection should be working again. Of course, VoIP will still be out.

11. Log into gizmo again as admin. Go to VoIP->SIP Proxy. SIP Proxy will show as 0.0.0.0 ; change to 72.165.163.34:10560

12. In Preferred CODECs List, change 2nd entry from G729A/8000 to NONE. Click Save; Click OK to warning.

13. Go to VoIP->User Account. Enter your data for User ID, User Password, Confirm Password, User Name, Authentication ID.
Note: User ID and User Name are your 10-digit SR Phone number. Authentication ID is 15 digits. Password is 24 characters. The quotes are not part of the password, but the two equal signs at the end are. You should paste this data into both the User Password and the Confirm Password fields, as entering it manually is very error prone.

14. Set Enable T38 FAX to No. Click Save. Click OK to warning.

15. Go to Management->Reboot. Click OK to warning.

16. With luck, you should get a VoIP light within one minute.

Notes to other users who may try this:
If your SIP password contains one or more + characters, there is a bug that prevents you from entering it via the Inno web interface; use telnet instead (Cu command).
If you are in the US, note that this setup will not receive updates from SR; your service may stop working if a critical update is pushed, until you make it manually. Be sure that you have a reliable alternate means of making an emergency call.
All of what people said here (on the blogspot place) failed for me. I tried to combine a few and see if it worked.

Finally, I got this to work.

This is after unplugging the Innomedia from WAN and connecting it from LAN to my machine.

1. Use a paper clip to do the RSTR thing on it. I did it several times just to be damn sure. I pressed RSTR for excessive amount of time after the router had started. Also, kept it pressed for an excessive amount of time as I plugged the power into the Innomedia thing. (I wanted to make sure I did get a proper hardware reset)

2. Next I tried the "Admin"/"password" thing the last guy posted. It did not work. I tried "admin"/"password". It did not work. Same problem with "user"/"welcome" on both the links. None of the above & below actually worked before I did the RSTR heavily.

Then I tried "admin"/"slapshot". It worked.

I was finally in and thanking my starts.

3. The first thing I did was to go into the > IPNetwork > Provisioning and turned it off. It rebooted and I used the "admin"/"slapshot" thing again

4. The next thing I did was go to > Management > Administrator and change the login/pwd "admin"/"xxxxxxxxx" (xxxxxx being whatever password you want)

5. Now I am trying to figure out what the hell to do next

At last I have got my phone service working. I just followed the simple instruction from joely and set the provisioning address to 67.133.234.53:80 and kept provisioning flag turned on. I believe it took around 2 days to get it successfully provisioned. Yesterday night around 10 pm the voip line was off and run light blinking and I still had no phone service. But the only difference was that I could no longer log on. That means it got provisioned but somehow not connecting with their network. I just power-cycled it and voip light turn on. So just look for when your login fails. May be a power cycle after that will also solve your problem.
