The official policy is that ALL forwarded calls are charged $.039/minute regardless of whether they are outside the local calling area or the 200 minute level. In those previous threads this was discussed ad nauseum. If it was as you described then I would get the $15 plan. I even called VoicePulse to ask and they confirmed what I wrote here. Their website confirms it too. You might not incur such charges. Your situation then would not fit the official policy. If I switched to the $15 plan and was charged the forwarding fee I would have no basis to complain.
