You would get a bill itemized by what you were accessing - websites in category "A"; a websites in category "B"; Youtube videos; email; bittorrent or other filesharing; VOIP; etc. - and by the byte, by the minute, by the time of day; at various rates according to a complex, constantly-changing schedule - whatever they can think of.

And corporations are sociopaths, so the ISPs will certainly try whatever they can get away with if they think it will maximize profit.
