As SB said you stats from modems side look good. I just wander what is UPSTREAM SNR - unfortunately to get this you need to call tech support. Other thing might be short IP lease time. So check "ipconfig /all" for that. if it is a week time that we good if it's like an hour it might be an issue with network side -- and again you need to call Rogers to fix it. for trouble shooting connect one PC only straight to the modem.

You are losing signal. When working, your modem gets a good signal, but it lost it there and it looks like you did a power down and back up of your modem when you took that. (1970 dates).
Call Rogers there's an intermittent signal problem.
