Before I left my previous employer (a cable MSO) there was a good amount of headway into planning for IPv6 transitions and its gotten much easier thanks to the transition technologies finally being put into place. In case you aren't aware RFC1752 actually contains some transition specific criteria to help providers during the "mixed network" phase of the transition
