| Don't think it is IDSL. My understanding is IDSL uses ISDN signalling so it is limited to 144 kbps symmetrical.
There is a version of ADSL2+ reach extended able to deliver 768 at 19,000 and lower speed a few thousand feet further out.
I have no idea what kind of ADSL can deliver such high speed at 42,000 feet unless it is some form of 2-wire T1 (HDSL) able to use repeaters.
