I've been running the Zoom Telephonics Model 5654 X5 Ethernet/USB ADSL 2/2+ Modem on OS X and Linux for 2 years.

It's rock solid, extensible and support ADSL2+.

It's just over $50 currently. I bought it when it was $90.

The Model 5590CF Zoom X6 ADSL 2/2+ Modem w/ Wireles is the added Wireless 802.11g version with some standard additions.

Both fully support QoS for VOIP.
