Telling the name of the file is pretty easy. You are double clicking it in order to run it, correct? If "4200_upgrade.exe" is the filename then that is correct for Modems that end in A12 on the Bottom Label part number.
As to the Mode of the 4100, it is a Router by Default and using it in Router Mode means that you have to enter your Username and Password in the 4200 Web Interface at »192.168.254.254 so that it makes a PPPoE connection to your ISP.
In Bridge Mode means you have made extra effort and changed its function to no longer log in with your Username and Password then also either set your PC to dial a Broadband Connection using a PPPoE Dialer (with your Username and Password) OR you have added a separate Router behind the 4200 made by another company (Linksys, D-Link, Cisco, ZyXEL, 2Wire, Netgear, Belkin, Westell, Buffalo Tech, Motorola, Netopia/Cayman, SMC Networks, 3COM/USR, ETC.) that is making the PPPoE connection when you have set it up with your Username and Password instead of the 4200. If you do have a Router between the Modem and the PC then it must be removed and you have to connect your PC directly to the 4200.
