Do an online search of the templates on Microsoft's Office section, there are some really good ones in there that are Excel sheets that are formatted nicely, easy to use and easy on the eyes.
If you want something web-based, (PHP, MySQL), check out cInvoice from ForPerfect. $40 (approx) for the life-time license.

Also, MS Small Business Accounting (I think that's the name) has invoice-making abilities in both the free and paid versions.
