Setup is very similar to Vonage -- CallVantage (CV) runs on any broadband setup. AT&T ships you a TA and you plug it into your DSL/Cable/Fiber/Other broadband internet connection, and the plug your phone into the phone line port.

866-596-8464 - CallVantage Customer Service. You must call to order to bundle with DSL or U-Verse, and of course you can order via phone unbundled or bundled with AT&T Wireless as well.
As far as U-Verse Internet only, some users have gotten it to work and others have not. But the process is order TV + Internet, and then cancel the TV sometime during that first month of free TV. Some users have gotten to keep their U-Verse internet after the canceled TV, and some have not.
