Put bluntly, your line is crap. 63.5 dB attenuation is the culprit.

Try to connect the modem directly to the jack in the network interface, thus removing all of your inside wire and whatever is connected to it, and pull those stats again. If they don't improve there isn't much you can do. If they do improve, then you have some inside troubleshooting to do.
