I went into the second set of menus, and I found the "set - picture format" but the only options available for that were "Fixed, Auto DVI, Up-Convert 1, and Up-Convert 2. If I select any of those settings besides FIXED, the box displays "AUTO" next to 1080i. However, I did not see any option for "Pass-Through" within that menu.

as some others previously said, you probably need to perform the SA Explorer HD Setup. To launch the HD Setup Wizard, press the "INFO" and "GUIDE" buttons directly on the cable box (at the same time) while the cable box is OFF. The TV should be left ON while you do this.
It should come up as "TV Connection Set-up" and you would hit the "B" labeled key on your remote to use HDTV mode.
Then it should say "Setup Options" "A" button for easy setup "B" for advanced **USE THE "B" option so that you can change it out of 1080i
Once you go into Advanced setup, choose 16:9 for widescreen and follow the instructions. Pay attention to the bottom of the screen where the different formats (480, 720, etc..) are listed. The ones that have a blank dot are NOT ENABLED the ones with a green dot ARE ENABLED. As you go thru the steps it will test each one and you will tell the box which ones you can see. The box will enable and put a green dot next to the ones you tell it you can see. Be patient, as it will go thru each one at a time.
Once you are done selecting the formats press "EXIT" and then "POWER" and then try the quick settings when you go to the HD Channel you want to change, and you will find the picture format, same thing for the main menu. (See my previous posts)
