They've charged me both ways over the years. It apparently depends on the other devices and services you have. With the DVR, I paid $5 per additional outlet; without the DVR, I pay $1.50 extra for the second CableCARD in the TiVo. There are evidently certain combinations in the computer, and they need to fit your services into the pricing they have set up.
