Fed Up with Cavtel, need a little help

Switching my business (Phone and DSL) from Cavtel to Verizon after months of horrible service.

Cavtel requires that I fax them a "Move form" which, of course, they do not tell you where to locate. Does anyone know where I can download the form? It would really help.
