When I first signed up for Teliax, they didn't even have CNAM for that price, but they do now, not that I really care. The number is all I really need.
Other providers that go the Teliax route of charging per minute on DIDs usually only charge $2 or $3 a month and 1c a minute, which seems more reasonable, given what they are paying for the DID.
Again, my problem with the pricing isn't the $4.99 a month in and of itself (after all, I'm happy to pay twice that for a number in Rio, although that is flat rate), it's the $4.99 a month combined with 2c a minute incoming. If they dropped it to 1c a minute, I'd be happier.
As far as their international rates are concerned, to most destinations they're out to lunch, but as I mentioned in the review, their reliability makes them a reasonable backup to another less established provider, even when they charge 20c more a minute to many destinations. -- It's wierdo, not weirdo.
