His monitor is 75hz so vsync should only cap his framerate to 75. My guess is the video card is fairly new, so perhaps the drivers haven't reached full maturity.
I've had a lot of ATI based cards, and I've seen "fixed frame rate issue in game xyz" or "improved frame rate by 50% for cards xyz" in the patch notes fairly often.
Having said that, getting 30 FPS in a major city like Shatt is actually not too bad. I'd expect the full 75 FPS in Stormwind or Undercity, but Shatt is pretty infamous for crippling frame rates.
I suspect it has something to do with the population, and simply the sheer amount of stuff going on in most areas. It's the same thing in 25 man raids, when things start to really kick off you sometimes see frame rates dip to 30 - 35 FPS even on good systems. |

Doesn't really change my main point, which is Shatt and high population areas are most likely the cause of the lower frame rate. If he had dual video cards and the best processor / ram possible then vsync might be nerfing his frame rate in Shatt. But it's unlikely with that setup. |

But if the computer cannot produce the 60 fps, because of vsync it will only display full complete frames. Because of that, it will display only 30fps because it doesn't have a new frame ready to feed the monitor every cycle, so you get the same frame twice if it is putting out between 30 and 59fps. |
