Well I think that is RJ25 or something, but not here to debate that kinda junk.
I think the OP needs to be a bit more specific on what they meant by plug vs. jack. Are they trying to run the homerun from NID to a wallplate?
You can probably find all you need at home depot or lowes and if you can find the parts to buy you can read the package on how to do it. Not rocket science.
