When you look at the other countries on the list (with the exception of Sweeden really) most of the countries are quite small, therefore the population is more condensed, and everything is easier to wire. Its tough and not-money smart for US-ISPs to get broadband to rural folks in wyoming

It's a combination of multiple factors.

1) Population density
2) Government built telecom (NTT Japan was government built, still partially government owned. Similar situation in South Korea)
2) Investors willing to take on long term (30+ year) debt

Within the US cities you mentioned you'll still find pockets of places with better access. A friend of mine lives in NYC and his building provides 100mbps Ethernet access to all of the condos. Out on the west coast you have services like Paxio.

It's not that the services don't exist in the US -- they're just not widespread.
