RJ45
Attached is the best photo of it I could take without moving furniture.

The other box on the right is the standard RJ11 which is unused. As you can see mine was installed inline with the RJ11. My understanding of what AT&T told me was that I'd need an RJ45 for each jack i wanted to use with the ISDN. But that may (or may not) have anything to do with my house having 50 year old wiring in it. The tech that installed my ISDN had to install a second demarc box outside.

It might be possible to run it over the existing phone jacks but that's not something I can guarantee will work. Maybe someone else can give you a clear answer on that. I just know that my ISDN only works on the one RJ45 jack. The RJ11 that it's connected to is POTS still.
