Just for the benefit of other members and FiOS subscribers, I'm posting excerpts of a chat I just had with FiOS technical support (not through the site).
The short version: Don't be afraid to push just a little bit - even test tools make assumptions. Both the tech and I learned something today!

VZ Tech: ok it is set properly in the server VZ Tech: so VZ Tech: lets trouble shoot there VZ Tech: first power cycle the router then run speedtest again VZ Tech: what is the OS of the computer and it is hardwired to router correct? Raydr: there's nothing wrong with the router or any of the PCs. I have tested with multiple routers and multiple PCs. I can get 20Mbps upload Raydr: i just can't get faster than 10Mbps download Raydr: i have used your optimizer program as well, multiple PCs Raydr: I have attempted 4 different routers, including yours, as well as a direct PC connection to the ONT with multiple PCs Raydr: there is something on your end VZ Tech: how are you testing speeds? Raydr: using both speedtest.net and the verizon speedtest site VZ Tech: ok test again Raydr: testing, using VZW site... VZ Tech: then power cycle router and test again Raydr: upload, 21942 kbps Raydr: download, 10.061 Mbps Raydr: using speedtest.net, got 10.04Mb/s download, 15.63Mb/s upload VZ Tech: can you switch out the wan ethernet cable Raydr: I'm on cable #2 for WAN...but as you can see, I'm able to upload at 20Mbps so downstream traffic should be able to reach that speed too. Raydr: i really don't need the 20Mbps, i'm fine with 10, can we just downgrade my account to 10/10? Raydr: so I'm not paying for it VZ Tech: checking to see if we can do that internally.. a network tech is still looking at issue tho, so hopefully we can get you fixed or get the chage done Raydr: no problem,i'll wait to see what the network tech says VZ Tech: NT found issue Raydr: i'm a techie, would love to know what it is/was VZ Tech: BAAIS says 20/20, but the GWR says 10. anne's going to investigate and send a ticket up to Baais Tier 2. Open an MCG ticket and toss me the number VZ Tech: BAAIS is prov server VZ Tech: whayt I can see VZ Tech: I could not see GWR and my test that test your speed on your router is pass fail return VZ Tech: so since it thought it was suppose to have 10 it said pass VZ Tech: not sure why prov server did not pass it to the GWR Raydr: i see. i'm surprised the gwr supports a 10d/20u configuration. is downstream and upstream provisioned independently? VZ Tech: yes Raydr: i see...so I guess the downstream provisioning failed. cool, glad you guys found it. VZ Tech: glad you pushed me to Raydr: I was a cable modem installer, then a satellite installer for years, kind of learned over time to segment issues. is this something that will take a day or two to resolve or is this a real time thing? VZ Tech: real time VZ Tech: actually let me verify that VZ Tech: run a speed test for me Raydr: lost internet for a moment, must have been them reconfiguring. running now Raydr: it's working! VZ Tech: awesome, so I learned some new stuff today VZ Tech: did not know that te prov server was not an active link to rgwr to verify it is set correctly VZ Tech: see what happens when people assume Raydr: lol, yep! i think we can close this one out! That is one really cool, informative troubleshooting chat session you posted, Raydr.

The way I read it, the provisioning server (BAAIS) said 20/20 which is what the FSC tech can see. He can click a button which tests provisioning at the gateway router (GWR), but the result is simply pass/fail. The GWR was set to 10 download (in error), which is why the test came back Pass.
The GWR is where the action is. Your actual provisioning is governed there. The NT has access to that, which is why it took the second person to find and fix the problem.
Interpretation based on limited knowledge above, pure speculation below.
Since it is possible to release the WAN IP and acquire a different address, I think the process involves a lookup at BAAIS to get the operating parameters at the GWR. Once set, there's no need to look it up again. To fix it, the NT may have released the connection, forcing a new connection (and lookup).
