I may be wrong, but I've always thought of it as ZonedOut is the Interface (GUI) and IESpyAd contained the actual block list.
I would set my systems up using ZonedOut with Eric's list from IESpyAd

I'm not sure why you think that things have changed.
IE-SPYAD is the list from Eric.
ZonedOut is the tool from funkytoad to put Eric's list in the right "place".
As far as I know that has been the case for quite some time now since Eric's old IE-SPYAD with the regfiles was abonded.

BTW, some remarks:
1.

I think some things have to be updated there.
For example: for ZonedOut it gives:

I think it is now:

2.
ZonedOut site:
For IE8 it says:

I thought that that issue was more or less fixed with the latest IE update.
