I just used that last month to get a new license. It took about two weeks to arrive in the mail and was relatively painless.
The only frustrating part was, my dog is neutered but since I couldn't find my old file in the system I had to pay the un-neutered rate, which is $3 more. I could submit a notarized affidavit but the notary would charge as much as the un-neutered surcharge.
Or if you are in NYC you can go to your county clerk office and get it notorized for free.
Really wouldn't be worth saving $3 then, would it.
