If the problem is STILL present even at the NID (You plugged it directly into the test jack, right?) then aside from a potentially faulty DSL modem, bad grounding somewhere or a bad phone cord, the problem is on AT&T's side.

As for the impulse noise, anything from a bad dimmer light to bad grounding somewhere can cause this issue.
