I just installed nspluginwrapper v1.2.2. Youtube flash now successfully plays in full screen & HQ.

I did the same thing, and it solved the problem for me as well! I have no idea why, to be honest, but YouTube works in full screen again! Thank you, all! Perhaps in a later update to Firefox, there may be a patch that solves this, and I can then remove/disable nspluginwrapper. Until then, this works for me!

Found another solution that works for me -

First uninstalled nspluginwrapper. Then added the following to '/usr/lib/firefox/firefox.sh'

export LD_PRELOAD=/usr/lib/libGL.so.1

So the script went from this:

Works fine with full screen flash, as did nspluginwrapper. The only issue may be when upgrading Firefox. If the problem persists the above line will need to be re-added to a newly installed FF script.

I suspect that this is a new video related issue, not necessarily limited to Nvidia, perhaps stemming from FF 3.5 - HTML 5 capabilities.
