Slow/Intermittent Connection in Chandler

As of three days ago our internet has been completely useless here in Chandler, AZ.

As far as the modem's signal page goes (all with a computer hooked directly to modem):

Downstream:
Frequency: 579000000 Hz
Signal to Noise: 36dB
Power Level: -3dBmV

Upstream:
Frequency: 37808000 Hz
Ranging Service ID: 5013
Symbol Rate: 2.560 Msym/s
Power Level : 50 dBmV

I did the speed test on speedtest.net and received the following results:

Download: 4.54Mb/s
Upload: 0.07Mb/s

The actual cable setup is pretty messed up in this house, but this issue has only recently presented itself, with no corresponding changes made w/in the home or with equipment. The only way I am able to access the internet right now is directly connected to the modem, using the router nothing works at all.

So, I guess I'm wondering if anyone sees anything wrong with the signal values or is having similar problems in Chandler (Near Ray and the 101).

I have a Surfboard SB5120

and the bill is says "Cox HSI Preferred W/ Powerboost"

not sure exactly what tier that would be, I am living with a bunch of girls and they set it up..

Preferred is the midrange tier and is like 9000/768
