1 edit | OK, something is messed up/mis-configured with this 5200. I suggest resetting it to factory defaults and then trying the DMZ settings again.

1 edit | OK, I don't know what to tell you as it is supposed to use "admin/admin" for the username and the password according to their own FAQ here.

Note: If you are not currently logged into your modem, you may need to enter your modem's username and password (typically, admin is both the username and password).
Are you positive it was reset? You would have to login and re-enter your email address and password for the PPPoA connection to connect to Frontier. Did you have to do that after the reset or not?
You should be prompted after the reset is successful to login and the Modem will be unconfigured so that you have to enter your Frontier.Net Email address and Password to get the Modem to connect. Otherwise you will not have access to the Internet until it is configured again.
 After a successful reset you should be prompted to gain access to the Modem like above.
 Once you are back in the Modem, you should see that the Username and Password are blank waiting for you to fill them.
Otherwise the reset was not successful and you need to try again until you get it right (usually 4 seconds and let go).
