I'm guessing Comcast hasn't fiddled with it because

a) 99% of their user base doesn't go above 250GB (I'm a power user and average 100-200GB)
b) Business tiers are very affordable (they don't cost much more than residential) and don't have caps
c) In most areas people will pick Comcast (speed with caps) over the competitor (slower without caps). Especially when the slower rival has subpar service. Meet Qwest in Golden for an example of that one.

Qwest actually has caps - they just aren't explicit about how large they are. There are a few scattered reports of people getting booted over it.
