Why google, when you can just go to USA.gov or Travel.gov and find out the same info.

this looks like for it to be for the passport and the card

I wish there was just some form for the card and that is it
Simply check US Passport Card Lost -OR- Stolen then that form will only be for the card and that is it.

If you want a new card you have to file Form DS-64 and Form DS-11
