Yeah I got one too. Note that it's an outside firm handling the survey.
That's not an uncommon practice. In my business, we use an outside vendor for our customer satisfaction surveys all the time.

I burned them. Two years after they POd me with their crappy "briteport" modem I am still with Speakeasy. I'm in a location where the choices are few, especially after the merger I found a cheaper vendor, but was hoping to increase my speed rather than save money.
I noticed you can't even get pricing on the speakeasy website anymore. A few years ago I caught them dropping the price of my plan and not notifying me. I complained and got my DSL reduced $10 a month.
