It is not necessary or even desirable for trunks to have unique IP addresses or ports. In your example, incoming calls should come to "extension" 6185551212 or 6185551213, according to which number was called. I'm not familiar with FreePBX, but it seems likely that gets used when you specify inbound route caller ID.
