subsonic port forwarding issues

application is subsonic, modem is 3800HGV-B, did as port forwarding stated and opened up to app specific and opened port 80 and 4040 but when I go to subsonic host this is what I get...

Thanks in advance im sure its a dumb problem because its worked for 2 years until now.
