That could be true with internet but with TV your statement is not accurate. Anyplace in the US you have the option of cable, DishNetwork, DirecTV and in some markets FIOS and in much less markets uverse. I call that fierce competition, mainly from the satellite companies.

When it comes to internet that is another story. Cable was/is everywhere and the telcos response was ADSL. ADSL worked for many years and the telcos had plenty of time to expand. Today FIOS expansion is practically dead, uverse is practically non existent due to distance restraints. So who's at fault here?

If the cable companies are making tons of money then they must be doing something right.
