West Orange, NJ | Premium Channel Rebate Offers I noticed on the Starz website they have a $25 cash back offer for new subscribers. You have to mail in a copy of your last 3 bills showing you are a new Starz subscriber. I was going to do this, but my bill only shows Ultimate HD, and does not specifically list Starz even though it is part of the package. Has anyone been successful with this type of offer? I subscribed to Ultimate HD last fall so I should qualify as a new subscriber. I believe Showtime has a similar $25 rebate offer. |

Include a printout from the verizon website showing what channels are part of the ultimate hd package, circle the premium channel that you are requesting a rebate for.
Also circle the line item for the ultimate hd package on each bill.
Hopefully this will show that you are a subscriber to the channel offering the rebate.
