Don't know what a '9145 Network Interface Device' is...

3900-series router is next, I'm guessing?

HP Procurves, followed by UPSs?

What's this serving if you don't mind my asking?

The 9145 is our 100Mb hand off from Verizon.

Correct, Cisco 3900. No modules at the moment just basic routing, but that should change shortly.

Correct. (1) HP 5308xl, (3) HP 2650, (1) HP 2848.

(1) Old Amer.com media converter chassis.

Its the MDF for a school campus.

Above are several fiber RICs. To the left is a 42u APC server cabinet and to the right is (3) 42u two post racks with patch panels for data, PBX, and security. Serves about 20 buildings.

Only thing I'd pick on would be the fiber loom but it's not that bad and nothing is really stressed from the looks of it.

Do you have the fiber in duct as it runs through the rack and cable management ?

That my only question on this job. I hate Velcro but it looks like you didn't messy it up so good job.

The SM fiber from Verizon is in duct, but the rest is currently not. It is however in an almost totally isolated section of the vertical wire manager.

As an addon for that fiber loom.

That piece of equipment looks to be a serious PITA to work with since you can't really fit in tie bars or anything to hold the fiber at each row.
