I use "medium", also, as I depend on email to connect with clients and don't want to put the hurdle of requesting that authentication step in front of potential new income. Also, I get mailing list digests and other machine-generated mail that would not get through that step. It's good to be able to catch false positives in "known spam" and send them back to inbox, then on to my POP email clients. I just noticed what seems like an uptick in truly spam in "known spam" the last day or so. I don't think it has anything to do with Earthlink changing filters as they don't silently delete anything in "medium", afaik.
