Those are generally the only two things that cause issues such as what you have going on. Usually what happens is there is a tap problem and your signals go nuts and cause all the problems and your amp (which HSI isn't supposed to be running thru anyways) amplifies the problem. Also less frequently, bad outdoor splitters or bad cables. You have covered all the bases on this one.
