You only need to add them to the repeater router for clients that want to connect to the repeater router. I guess it doesn't hurt adding more than necessary.
Yes you'll need to add the repeater to the list on your host router.
You might want to turn off mac filtering initially to get it set up, along with any other firewall/filtering/security rules you have. Then turn them back on after you see they are linked.
