[Word] [2010] Generate QR code from text field

I'm trying to generate a QR code from multiple text fields. I have a text field called ITEM, DFU_DET_ASSN, and MODEL. The data would look like A-12345-12 JBALL SPH-M820.

I'd like to generate a QR code from whatever data is placed in these boxes. This would allow us, in the future, to quickly scan QR codes and generated the police item number, the detective who processed the device, and the model of the device.

Is this possible? If so, can somebody walk me through setting this up?

What are you using to create the QR code from the input in those text boxes?

A Google search revealed "StrokeScribe" but I can only get it to generated barcodes.

Any recommendations on QR add-ins would be helpful too.
