[AZ] Intermittent loss of phone and internet

Hi, I have bundled services from Cox. Over the last one month, have had intermittent loss of both phone and telephone, though TV works. Had 3 visits by Cox techs, have replaced the modem, spliter, checked outside lines but the problem continues.
Even during the loss the modem, status readings show downstream signal/noise of 38.5db in channel 1,2,3,4 and 45.5dbMV upstream in channel1, 0 in channel 2,3,4. Upstream signal/noise is 0 in channel5,6,7,8. Power level channel is -0.7, channel 2 is -0.3, channel 3 is -0.5, channel 4 is 0.1 and 0 in channel 5,6,7,8. Modem is Cisco DPQ3212 and phone is via the modem.
Is the inside line the problem. Appreciate any input.
