Are recommended updates usually released out of band?
This just looks like a OXPS to XPS converter tool but it's rated as important/recommended... 
Does it "ACTUALLY" require a restart?
EDIT: Restart requirement
You do not have to restart the computer after you apply this update.

Microsoft releases security updates on the second Tuesday of every month and non-security updates on the fourth Tuesday of every month. (Sept 25)
